Below are the instructions for completing the Project/Performance Site Locations webform. 

If there is more than one performance site, including any Department of Veterans Affairs (VA) facilities and foreign sites, list them in the fields provided for “Location 1” by clicking the “add location” button.   

If you need to add more than 30 locations, enter the information in a separate file and click “Add Attachment” in the “Additional Locations” field.   

Do NOT check the “I am submitting an application as an individual, and not on behalf of a company, state, local or tribal government, academia, or other type of organization” box.   

Project/Performance Primary Site Location Form

Organization Name 

Enter the organization name of the primary site where the work will be performed. 

UEI Number 

Enter the UEI number associated with the organization where the project will be performed. 

Street1 

Enter the first line of the street address of the primary performance site location.  

Street2 

Enter the second line of the street address of the primary performance site location.  

City 

Enter the city of the primary performance site location.  

County/Parish 

Enter the county or parish of the primary performance site location.  

State 

Select the state of the primary performance site location.  This field is not active until USA has been selected for the country.   

Province 

Enter the province of the primary performance site location.   

Country 

Select the country of the primary performance site location.  

Zip Code 

Enter the 9-digit postal code of the primary performance site location. 

Project/Performance Site Congressional District 

Enter the Congressional District in the following format:  2-character State Abbreviation – 3-character District Number.  Examples:  CA-005 for California’s 5th district, CA-012 for California’s 12th district. 

If all districts in a state are affected, enter “all” for the district number.  Example, MD-all for all congressional districts in Maryland. 

If nationwide (all districts in all states), enter US-all. 

To locate your congressional district, visit the House.gov website.  Note, it is likely this field will be identical to the “Congressional District of the Applicant” field provided elsewhere in the application. 

For state and U.S. territories with only a single congressional district, enter “001” for the district code.  For jurisdictions with not representative, enter “099.”  For jurisdictions with a nonvoting delegate, enter “098” for the district number.  Example:  DC-098, PR-098.
